   Submit your application at jobs.utah.gov/mycase (myCase), the same portal DWS uses for SNAP, Medicaid, and child care assistance.

   Or apply in person or by phone

   Visit your nearest DWS Employment Center (find one through the online Office Map), or call 801-526-0950 locally or 1-866-435-7414 toll-free to have a paper application mailed to you.

   Submit by mail or fax if needed

   Mail to Imaging Operations, P.O. Box 143245, Salt Lake City, UT 84114-3245, or fax to 1-877-313-4717 (toll-free) or 801-526-9500 (Salt Lake area).
